Reimbursement requires physician order, department determination of medical necessity, parent or guardian informed consent, and milk from a donor milk bank meeting department quality guidelines

Electronic web-based prior authorization system using best medical evidence and national care standards shall verify medical need before reimbursement

Department must promulgate rules for implementation including physician documentation requirements and informed consent form procedures, subject to chapter 536 rulemaking provisions

Rules proposed or adopted after August 28, 2014 shall be invalid if general assembly's rulemaking review powers under chapter 536 are held unconstitutional
