Requires voters to present photo identification to vote, limited to: non-expired Missouri driver's license, non-expired Missouri non-driver's license, U.S. or Missouri government-issued photo ID, or military/veteran ID from the National Guard, Armed Forces, or VA

Voters without photo ID may cast a provisional ballot by signing an affidavit if they cannot obtain ID due to: physical/mental disability, inability to pay for birth certificate or supporting documents, sincerely held religious belief, or being born on or before January 1, 1949

State must provide free photo ID to qualified voters who lack one and cannot afford it; all election authority costs for implementing photo ID requirements must be reimbursed from state general revenue

Voters who cast provisional ballots due to lack of photo ID have three days after the election to return to the election authority with proper identification to have their ballot counted

Act becomes effective only upon voter approval of a constitutional amendment authorizing photo identification requirements for elections
