Allows capable adult Missouri residents with a terminal disease (prognosis of death within 6 months) to request prescription medication to end their life after making two oral requests at least 15 days apart and one written request witnessed by two individuals

Requires confirmation by both an attending physician and consulting physician that the patient has a terminal disease, is mentally capable, and is acting voluntarily; mandates psychiatric/psychological evaluation if either physician suspects impaired judgment

Protects health care providers from civil, criminal, or professional liability for good faith participation, while allowing providers to opt out of participation; permits health care facilities to prohibit the practice on their premises with advance written notice

Classifies forging a request, concealing a rescission, or coercing a patient to request life-ending medication as a class A felony

Requires the Department of Health and Senior Services to annually review records, collect compliance information, and publish statistical reports; specifies that actions under the act do not constitute suicide, assisted suicide, or homicide under Missouri law
