Requires entities whose primary purpose is providing pregnancy-related services (prenatal sonography, pregnancy tests, pregnancy options counseling) to disclose whether they provide FDA-approved contraceptives, onsite practitioner consultations, adoption services/referrals, or abortion services/referrals

Exempts entities that employ licensed practitioners (physicians, nurse practitioners, physician assistants, registered nurses, or nurse midwives) who are onsite whenever medical services are provided

Mandates disclosures be conspicuously posted at entrances, waiting areas, websites, and advertisements, and provided in a manner a reasonable person can read and understand before accepting services

Prohibits covered entities from disclosing patron health information without written authorization and requires they provide patrons one free copy of their health records upon request

Establishes administrative penalties enforced by the Department of Health and Senior Services: $250-$1,000 for uncured violations after 5 days, up to $5,000 for violations continuing past 7 days, and up to $10,000 per week for violations continuing past 14 days
