Requires physicians to obtain notarized written consent from both the minor and one parent or legal guardian before performing an abortion on an unemancipated minor under age 18

Mandates physicians collect and retain government-issued ID and documentation proving the parent/guardian relationship, keeping records for five years past the minor's age of majority or at least seven years

Establishes a judicial bypass process where minors can petition for a waiver if they demonstrate by clear and convincing evidence they are sufficiently mature and well-informed, or are victims of physical/sexual abuse by a parent/guardian

Creates a detailed standardized consent form that must include descriptions of surgical/medical procedures, comprehensive lists of risks and hazards, and requires the minor to initial acknowledgments that the procedure will end her pregnancy and result in the death of her unborn child

Provides a medical emergency exception where consent is not required if there is insufficient time, but the physician must notify the parent/guardian within 24 hours orally and by certified mail
