Establishes a statewide Prescription Drug Monitoring Program administered by the Department of Health and Senior Services to track prescribing and dispensing of Schedule II, III, and IV controlled substances

Requires dispensers to electronically submit detailed information within seven days of each dispensation, including DEA numbers, patient identification, drug name/quantity, and prescription details

Restricts access to the confidential database to authorized parties including prescribers, dispensers, law enforcement with subpoenas, licensing boards, and the state board of pharmacy

Imposes a $100 administrative penalty per violation for dispensers who knowingly fail to submit or submit incorrect information, and makes unauthorized disclosure of database information a Class A misdemeanor

Includes a referendum clause requiring voter approval at the November 2016 election or a special election called by the governor before the act takes effect
