Directs the Department of Social Services to seek federal approval to operate Missouri's MO HealthNet (Medicaid) program under a block grant model

Creates the "Healthcare Transformation Trust Fund" in the state treasury to receive federal block grant money and other non-general revenue funds, with remaining balances not reverting to general revenue at the end of each biennium

Establishes the trust fund as the sole payment mechanism for MO HealthNet obligations; if funds are insufficient, all provider payments must be reduced equally on a pro rata basis

Creates a 10-member joint legislative committee to set rules for covered benefits, eligibility requirements (including work participation for adults), expanded coverage through private option plans, and cost-sharing obligations

Requires biennial economic impact studies by the University of Missouri beginning in 2020, and includes a sunset provision if federal medical assistance percentages under the Affordable Care Act are changed or repealed
