Facility-based physicians who bill out-of-network patients more than $500 (after co-payments, deductibles, and coinsurance) must include a conspicuous, plain-language explanation of the mediation process on billing statements

Patients may request mediation for out-of-network billing disputes when the amount owed exceeds $500 and the services were provided at a hospital that is in-network with their health plan

"Facility-based physician" is defined as any Missouri-licensed physician granted clinical privileges by a facility who provides services to patients under those privileges

The Department of Insurance, Financial Institutions and Professional Registration may promulgate rules to implement the mediation process

Rules adopted under this section after August 28, 2016 would be void if the General Assembly's rulemaking review powers under Chapter 536 are held unconstitutional
