Adds numerous synthetic opioids and fentanyl analogues (including acetyl fentanyl, furanyl fentanyl, cyclopropyl fentanyl, and broad "fentanyl-related substances") to Schedule I controlled substances, and updates Schedules II–V with additional drugs such as carfentanil, thiafentanil, lorcaserin, and FDA-approved cannabidiol products.

Creates new drug trafficking thresholds specifically for fentanyl and carfentanil: more than 10 milligrams triggers first-degree trafficking (class B felony) and second-degree trafficking (class C felony), with 20 milligrams or more elevating both offenses to class A and class B felonies, respectively.

Reduces the 30-day purchase limit for over-the-counter pseudoephedrine/ephedrine products from 9 grams to 7.2 grams, adds a new 12-month limit of 43.2 grams, prohibits local governments from requiring prescriptions for these products within the statutory limits, and voids any existing local prescription-requirement ordinances enacted before August 28, 2020.

Prohibits edible marijuana-infused products sold under Missouri's medical marijuana program from being shaped like humans, animals, or fruit; requires packages containing 10+ milligrams of THC to bear a standardized diamond-shaped "THC/M" symbol in red and white; and directs the Department of Health and Senior Services to adopt rules prohibiting products designed to appeal to minors.

Requires all owners, officers, managers, employees, contractors, and support staff of licensed medical marijuana facilities to undergo state and federal fingerprint-based criminal background checks through the Missouri State Highway Patrol and FBI, with an emergency clause making this provision effective July 1, 2020, or upon passage.
