Violations constitute unprofessional conduct resulting in mandatory license revocation for health care providers

Creates a private right of action with minimum damages of $500,000, treble damages, attorney's fees, and a 15-year statute of limitations; plaintiffs who become infertile receive a rebuttable presumption of harm
