Removes the August 28, 2027 sunset date from Missouri's existing law requiring students to compete in athletics based on their biological sex as stated on their birth certificate at or near the time of birth

Prohibits private schools, public school districts, charter schools, and postsecondary institutions from allowing students to compete in athletics designated for the opposite biological sex

Allows female students to compete in male-designated athletics if no corresponding female competition is offered

Schools violating the law lose state aid under chapters 163 and 173, and affected students or parents may sue for injunctive relief and attorney's fees

Requires the Department of Elementary and Secondary Education and Department of Higher Education and Workforce Development to promulgate rules ensuring compliance with student medical information confidentiality laws
