Adds statutory definitions of "female," "male," and "sex" to Missouri law, defining sex based on reproductive biology (egg production for females, sperm production for males)

Requires covered entities (correctional centers, juvenile detention facilities, public buildings, public schools, and state higher education institutions) to designate multi-occupancy restrooms, changing rooms, and sleeping quarters for exclusive use by one sex

Creates a private cause of action allowing individuals to sue covered entities for declaratory and injunctive relief, plus attorney fees, if they encounter someone of the opposite sex in a sex-designated facility or are required to share sleeping quarters with the opposite sex (2-year statute of limitations)

Removes the August 28, 2027 sunset provision from existing law prohibiting students from competing in athletics designated for the opposite biological sex, making the restriction permanent

Includes exceptions for custodial duties, medical assistance, law enforcement, emergencies, and athletic coaching, and permits accommodations for ADA-protected persons, young children, and elderly individuals requiring aid
