Creates the "Honest Billing Act" requiring off-campus outpatient departments of hospitals (located more than 250 yards from the main campus) to obtain and use a unique National Provider Identifier (NPI) distinct from the main hospital

Requires claims for health care services at off-campus outpatient departments to be billed on CMS 1500 forms or HIPAA X12 837P electronic transactions, effective for claims submitted after December 31, 2026

Prohibits facilities from holding patients liable for health care services if billing requirements are not followed, with violations constituting a violation of the Missouri Merchandising Practices Act

Authorizes the Department of Health and Senior Services to impose penalties including $1,000 administrative fines per violation, license suspension or revocation, probation, and requiring providers to post penalty information on their website

Grants enforcement authority to the attorney general and allows the Department of Commerce and Insurance to refer violations and promulgate implementing rules
