Creates "multidisciplinary adult protection teams" of two or more persons to facilitate identification, investigation, prosecution, prevention, and treatment of offenses against eligible adults aged 60+ or disabled adults aged 18-59

Teams may include representatives from prosecuting attorneys, law enforcement, medical/health providers, nursing homes, mental health providers, guardians/conservators, financial institutions, and community agencies

Authorizes team members to access confidential abuse and neglect reports and share information with each other when acting in their official capacity for the benefit of eligible adults

Allows disclosure of reporter names to the Department of Mental Health for investigating abuse and neglect of vulnerable persons

Unauthorized dissemination of confidential information from reports or the central registry constitutes a class A misdemeanor
