Establishes minimum reimbursement rates for out-of-network ambulance providers: either the rate set by local government in the jurisdiction where services originate, or 325% of the Medicare rate for the same service (whichever is less than billed charges)

Prohibits balance billing by requiring payment under these rates to be considered payment in full, except for enrollee cost-sharing amounts (co-payments, coinsurance, deductibles)

Caps enrollee cost-sharing for out-of-network emergency ambulance services at in-network rates

Requires health carriers to pay clean claims directly to ambulance providers within 30 days and prohibits sending payment to enrollees

Excludes air ambulance providers from coverage under this section
