Requires health carriers and MO HealthNet to provide mandatory coverage (changed from "offer" to "provide") for orthotic, prosthetic, and assistive devices including wheelchairs, custom-fitted orthotics, and prosthetic limbs for daily use, physical activities, and bathing

Mandates coverage of multiple devices per enrollee: one prosthesis/orthosis for daily use, one for physical activity (running, biking, swimming), one for bathing, plus one daily wheelchair, one backup manual wheelchair, and one activity wheelchair if medically necessary

Requires replacement coverage without "continuous use" or "useful lifetime" restrictions when a provider determines replacement is necessary due to physiological changes, irreparable device damage, or when repair costs exceed 60% of replacement cost

Prohibits discrimination in utilization review decisions based on actual or perceived disability, and bars denial of benefits to individuals with limb loss for services that would be covered for non-disabled persons seeking to restore the same physical function

Extends coverage requirements to Medicare supplement policies (previously excluded), requires health carriers to maintain at least two in-network orthotic/prosthetic/assistive device providers in Missouri, and mandates reporting to the insurance director on claims data by October 1, 2027
