Appropriates approximately $19.4 billion total for the Missouri Department of Social Services for fiscal year 2027 (July 1, 2026 - June 30, 2027), covering MO HealthNet (Medicaid), Children's Division, Family Support Division, Division of Youth Services, and administrative operations

Allocates over $5.2 billion for Medicaid expansion under Article IV, Section 36(c) of the Missouri Constitution, including $4.2 billion from the Title XIX Adult Expansion Federal Fund and $398 million from General Revenue

Funds major social services programs including $169.3 million for adoption and guardianship subsidies, $95.9 million for foster care maintenance payments, $44.8 million for Blind Pension benefits, and $16.9 million for TANF cash assistance

Provides $51.5 million for the Missouri SuN Bucks Summer EBT program, $37.9 million for refugee resettlement services, and $12.9 million for alternatives to abortion services including infant hygiene products

Transfers funds between various special funds and General Revenue totaling approximately $2 billion, including $718.7 million through Federal Reimbursement Allowance Fund and $210.9 million through Nursing Facility Reimbursement Allowance Fund to support Medicaid provider payments
