Requires MO HealthNet (Medicaid) to cover biomarker testing for diagnosis, treatment, management, and ongoing monitoring of diseases when the test provides clinical utility supported by medical evidence

Mandates health insurers, nonprofit health service plans, and health maintenance organizations to include biomarker testing coverage in policies issued or renewed on or after August 28, 2026

Coverage must be supported by FDA-approved or cleared test indications, CMS national or local coverage determinations, or nationally recognized clinical practice guidelines

Requires insurers to provide a clear, accessible process for patients and practitioners to request exceptions to coverage policies, available on the insurer's website

Excludes biomarker testing for screening purposes from required coverage under both Medicaid and private insurance provisions
