Requires the Department of Mental Health to develop a simple one-page affidavit form for commitment filings that affiant can complete without retaining an attorney, and prohibits clerks from requiring attorney representation.

Prohibits holding persons awaiting commitment in jail unless no reasonable alternative exists, and allows judges to dismiss affidavits without a hearing if examiners determine the person does not need treatment.

Establishes a $400.00 cap on total costs charged for all court proceedings incidental to commitment, and increases penalties for intentionally false affidavits to include criminal charges and contempt sanctions.
