Amends bail bond forms to require listing both the name and owner of the bond company posting the bond

Requires probation appearance bonds to display the name and owner of the company posting the bond

Mandates that bail bonds include preprinted or stamped information: full name of professional bail agent, Department of Insurance license number, legal address, and phone number

Establishes penalties for bail agents who fail to include required bond information: up to 6-month license suspension and/or $1,000 fine for first offense, up to 1-year suspension and/or $5,000 fine for second offense, and permanent license revocation for third offense

Effective date of July 1, 2010
