Requires psychiatrists, physicians, and psychologists to file written reports with the State Department of Health when treating patients for emotional trauma related to abortion.

Reports must be submitted within 30 days of patient discharge or death and include patient demographics, abortion method and date, facility type, treatment condition, and billing information.

Department of Health must develop standardized reporting forms, communicate requirements to medical organizations and facilities, and summarize aggregate data for the annual Vital Statistics Report.

Prohibits inclusion of patient names, social security numbers, and identifying information in reports; reports must be destroyed after 5 years; unauthorized disclosure of identifying information is a felony punishable by up to 3 years imprisonment or $5,000 fine.

Effective July 1, 2010; provides liability protections for physicians, psychologists, hospitals, and their personnel for violations except in cases of willful or reckless acts.
