Establishes a comprehensive bill of rights for patients in acute care facilities and residents in nursing homes and extended care facilities, protecting civil liberties and requiring written disclosure of rights at admission.

Requires health care facilities to provide courteous treatment, appropriate medical care, physician identity disclosure, complete information about diagnosis and treatment alternatives, and the right to refuse treatment and experimental research.

Guarantees patients and residents privacy in medical records and communications, freedom from maltreatment and nontherapeutic restraints, and access to grievance procedures and advocacy services.

Requires 30 days' written notice before resident discharge and 7 days' notice before room transfers, with exceptions for emergencies and utilization review determinations.

Permits physical restraints in nursing homes only with resident or guardian consent and physician order documenting medical necessity, with monitoring and periodic reevaluation required.
