Creates a voluntary statewide fictitious business name registration system administered by the Secretary of State to provide public identification of persons or entities conducting business under assumed names.

Requires registration applicants to submit the fictitious name, legal name and address, business locations, entity identification information, and a statement acknowledging no exclusive rights are conferred by registration.

Establishes a five-year registration period with renewal available during the expiration year; registrations expire December 31 if not renewed and may be amended within 30 days of material changes.

Sets registration and renewal fees at $25.00, withdrawal/cancellation/amendment fees at $25.00, and certified copy fees at $10.00, with exemption for entities that file with their annual reports.

Prohibits registration of fictitious names containing misleading terms or legal entity designators (corporation, LLC, limited, etc.) unless the applicant is actually organized as that entity type; imposes criminal penalty of up to $500 fine for fraudulent filings.
