Directs the Division of Medicaid to establish a medical home program that restructures payment systems to support primary care providers using multidisciplinary teams for care coordination, disease management, patient education, and home-based services

Requires the payment methodology to reward quality and improved patient outcomes, including care for patients with chronic diseases and the elderly

Defines eligible primary care providers to include general practitioners, family practice physicians, pediatricians, internal medicine providers, rural health centers, federally qualified health centers, and primary care outpatient clinics operated by general hospitals

Mandates implementation no later than September 1, 2011, subject to federal approval and availability of federal matching funds under Title XIX of the Social Security Act

Requires the Division of Medicaid Director to conduct annual evaluations assessing cost savings, health screening rates, chronic illness outcomes, and hospitalization/readmission rates, with annual reports to the Governor and Legislature
