Requires the State Department of Health to establish a "provider choice system" allowing healthcare providers participating in the child vaccination program to select any licensed vaccine, including combination vaccines and dosage forms recommended by the federal Advisory Committee on Immunization Practices

Limits vaccine cost to the department at no more than 115% of the lowest-priced equivalent vaccine, where "equivalent vaccines" must protect against the same infections, require the same number of doses, and have similar safety and efficacy profiles

Applies to providers in the Vaccines for Children program, local health departments, federally qualified health centers, rural health clinics, and other programs using state funds for vaccines

Mandates full implementation of the provider choice system by July 1, 2014, with partial implementation allowed earlier if feasible

Exempts the system during disasters, public health emergencies, terrorist attacks, military actions, or extraordinary law enforcement emergencies
