Requires affidavit ballots to include all previous addresses, present address, and at least one phone number (home, work, or cell) from voters in addition to complete name and telephone numbers.

Brings forward Section 23-15-719 prescribing the form and procedures for absentee ballot envelopes, including voter affidavit statements and certification forms for persons providing voter assistance.

Requires absentee ballot envelopes to display bold warning text stating votes will be rejected if the envelope is not signed by the voter and an attesting witness.

Directs the Mississippi Attorney General to submit the act to the U.S. Attorney General or U.S. District Court for the District of Columbia for Voting Rights Act compliance review.

Effective date contingent upon approval under Section 5 of the Voting Rights Act of 1965, as amended and extended.
