Requires the State Health Officer to establish a program to close health disparities across gender and racial groups in eight priority areas: infant mortality, breast and cervical cancer screening, HIV/AIDS and sexually transmitted diseases, adult and child immunizations, cardiovascular disease, diabetes, accidental injuries, and violence by 2016.

Mandates a statewide assessment using focus groups, needs assessment surveys, and key informant interviews to establish baseline data and measure the effectiveness of future funded activities.

Authorizes the State Health Officer to subcontract with a Mississippi-based entity specializing in health disparities and provides $250,000 in funding for needs assessment, outcome development, and program management.

Requires the State Health Officer to identify and select community-based organizations as planning partners and develop a comprehensive plan including morbidity and mortality data, assessment results, measurable outcomes, and a community grant program framework.

Mandates biennial evaluations and reports to the Legislature on program activities, grant fund usage, evaluation data, and outcome measures.
