• Requires Division of Medicaid to contract with a Care Service Organization (CSO) to develop and manage a network of primary care providers delivering advanced care and disease management services through a Medicaid Medical Home Shared Savings program.

• Mandates all Medicaid beneficiaries who utilize primary care services be enrolled in the Medical Home Shared Savings program and receive required physical examinations to establish baseline health and identify a medical home.

• Establishes cost savings sharing model where CSO receives 50% of savings and state receives 50%, with 50% of CSO's share distributed to primary care providers and 5% of state's share designated for primary care physicians education program at University of Mississippi Medical Center.

• Authorizes Division of Medicaid to immediately apply for necessary federal waivers and State Plan Amendments to implement the program and secure federal financial participation.

• Extends the repeal date of Medicaid services provision from July 1, 2012 to July 1, 2013, and removes the 15% beneficiary limitation on Medicaid managed care programs.
