Authorizes the State Department of Health to implement a physician choice system allowing healthcare providers in the Vaccines for Children program to select any licensed vaccine, including combination vaccines, that are CDC-recommended and available

Limits vaccine selections to those costing no more than 115% of the lowest-priced equivalent vaccine based on CDC price lists

Defines "equivalent vaccines" as those protecting against the same infections, requiring the same number of doses, having similar safety/efficacy profiles, and recommended for comparable populations

Restricts vaccine manufacturer visits to physician offices to one per month for educational purposes

Exempts the program during disasters, public health emergencies, terrorist attacks, or military/law enforcement emergencies, with full implementation required by July 1, 2013
