Requires that documents presented for recording include the name, physical mailing address, and telephone number of every grantor, grantee, borrower, beneficiary, trustee, or other party to the instrument on the first page below the three-inch margin.

Mandates that all required identifying information be placed on the first page of the document below the reserved three-inch margin along with the document preparer's name, address, and telephone number.

Establishes that failure to conform to format standards does not affect the validity or enforceability of the document or instrument.

Allows recorders to charge an additional $10 recording fee for documents dated on or after July 1, 2009, that do not substantially conform to the format requirements.

Effective date is July 1, 2013.
