Requires health insurance issuers and the Division of Medicaid to use a single, standardized prior authorization form for all prescription drug benefit requests effective January 1, 2014.

Limits the standardized form to a maximum of two pages in length, excluding instructions or guidance documentation.

Requires health insurance issuers to make the prior authorization form available electronically and allow prescribing providers to submit completed forms electronically.

Mandates health insurance issuers to file their prior authorization forms with the Mississippi Department of Insurance by January 1, 2014, and any subsequent modifications within 15 days prior to implementation.

Requires health insurance issuers to respond to completed prior authorization requests from prescribing providers within two business days of receipt.
