Authorizes and directs the Division of Medicaid to apply for federal waivers to develop consumer-directed options (CHOICES) for Medicaid recipients receiving home- and community-based long-term care services.

Permits eligible beneficiaries to select, direct, and employ persons to provide personal care services, homemaker services, in-home respite, and other unskilled support services.

Allows competent adults and caregivers to direct and supervise paid personal aides in performing health care tasks in their own homes, with physician approval.

Establishes that beneficiaries can manage individual home- and community-based services budget allowances through a fiscal intermediary based on functional assessment.

Requires the Division of Medicaid to promulgate rules defining qualifications, training, and oversight requirements for self-directed care tasks, developed with input from healthcare professionals and service recipients.
