Requires election managers to mandate any person providing voter assistance in federal elections or concurrent state/federal elections to sign a declaration form under oath before assisting.

Prohibits any non-election official from assisting more than 10 voters in federal elections or state elections held concurrently with federal elections.

Bars candidates, family members of candidates, and poll watchers from assisting voters in federal/concurrent elections, except candidates may assist their own family members.

Establishes similar but less restrictive requirements for non-federal elections, allowing family members and registered voters to assist (limited to 10 voters per assistant) with sworn declarations.

Sets penalties for vote fraud at up to 5 years in prison and fines up to $5,000, with declarations kept as public records available for inspection like poll books.
