HB 645 Summary

• Directs the Division of Medicaid to abolish all managed care programs, coordinated care programs, and coordinated care organization programs at the earliest possible date allowed under existing contracts.

• Establishes a "Physician-Centered Medical Home Program" as a Medicaid waiver program, modeled on the Community Care of North Carolina program, to provide integrated primary care with care coordination and management services.

• Requires minimum components of the medical home model including assignment to a primary care provider, health risk assessments, care coordination for complex cases, evidence-based clinical decision support, health information technology, and mechanisms to reduce unnecessary emergency department use.

• Directs the Division of Medicaid to apply for necessary federal waivers or state plan amendments from the Centers for Medicare and Medicaid Services and promulgate implementing rules if approved.

• Extends the repeal date of Section 43-13-117 from July 1, 2014 to July 1, 2015, and extends the repeal date of dental care provisions from July 1, 2014 to July 1, 2015.
