Establishes tiered pricing for paper copies of patient records: $20 for pages 1-20, $1 per page for pages 21-100, and $0.50 per page for pages 101 and beyond, plus 10% for postage/handling and $15 for off-site archive retrieval.

Sets maximum fees of $100 for electronic format copies (non-imaging) and $200 for imaging media in electronic format.

Requires medical providers, hospitals, nursing homes, and other medical facilities to provide written notice to requesting persons that records may be available in electronic format or imaging media before charging any fees.

Maintains existing provisions limiting physician deposition charges to normal, reasonable, and customary rates and capping medical record affidavit fees at $25.

Effective date: July 1, 2014.
