Reenacts sections establishing the State Board of Health with 11 members appointed by Governor, Lieutenant Governor, and Attorney General with staggered six-year terms.

Extends the automatic repeal date for the State Board of Health and State Department of Health from June 30, 2014 to July 1, 2017.

Extends the repealer on the certificate of need review fee (up to 0.25% of proposed capital expenditure, minimum $250, maximum $50,000) from June 30, 2014 to July 1, 2017.

Requires the State Department of Health to submit annual diabetes and obesity action plans to legislative committees by February 1 each year, including financial impact assessments, program evaluations, and detailed implementation budgets.

Extends the repealer on personal care home licensing requirements from June 30, 2014 to July 1, 2017, allowing residents to remain in personal care homes with physician approval even if skilled nursing services would be appropriate, up to two residents or 10% of facility capacity.
