Extends the automatic repealer date for the Qualified Health Center Grant Program from July 1, 2014 to July 1, 2019.

Maintains the total grant funding at $4,000,000 per state fiscal year with a maximum of $200,000 per center annually.

Requires grants to be used only for increasing access to preventative and primary care services for uninsured and medically indigent patients, including medical, dental, optometric, laboratory, diagnostic, pharmacy, nutritional, and social services.

Establishes an advisory council composed of two department employees, two qualified health center executives, two affiliated health care providers, and one unaffiliated health care provider to review and recommend grant awards.

Prohibits grant funds from supplanting federal funds, financing land purchases or real estate investments, or satisfying existing debt.
