Prohibits vision insurance contracts between insurers and eye care providers from requiring providers to offer services or materials at plan-limited fees unless those services are covered and reimbursed under the contract.

Requires providers to charge the same usual and customary rate for noncovered vision services as they charge for the same services to non-insurance patients.

Specifies that contractual discounts cannot reduce fees below what the plan would pay for covered services after deductibles, copayments, and coinsurance are applied.

Requires vision plan reimbursement for covered services to be reasonable and prohibits nominal reimbursement amounts used to claim services are covered.

Effective July 1, 2015.
