Establishes a Municipal Water, Sewer and Wastewater Systems Repair and Replacement Grant Program administered by the State Department of Health to assist municipalities with costs of repairing and replacing damaged, outdated, or inefficient water and sewer lines and wastewater treatment facilities.

Requires municipalities to submit a plan identifying proposed projects, specifying the condition of existing systems, and detailing the type of repair or replacement with design and specifications.

Requires municipalities to provide matching funds equal to 10% of the grant amount to be eligible for assistance.

Creates a special fund in the State Treasury called the Municipal Water, Sewer and Wastewater Systems Repair and Replacement Grant Program Fund, consisting of legislative appropriations and other designated monies, with grants made on a project-by-project basis.

Takes effect July 1, 2015.
