Allows parents or guardians to exempt children from specific vaccination requirements for school attendance by submitting a letter or affidavit documenting which vaccinations have not been given based on their beliefs.

Requires schools to accept an accompanying form (beginning 2015-2016 school year) that includes a health care practitioner's signed attestation that vaccination information was provided and a parent/guardian's signed statement confirming receipt of that information, both signed within six months before the child becomes subject to vaccination requirements.

Permits photocopies of the required form or letters from health care practitioners containing all required information and attestations as acceptable alternatives to the original form.

Authorizes school administrators to temporarily exclude unvaccinated children from school if there is good cause to believe they have been exposed to a disease specified for vaccination, pending local health officer clearance that the child is no longer at risk.

Defines "health care practitioner" as a physician, nurse practitioner, or physician assistant licensed to practice in Mississippi.
