Requires the Division of Medicaid to develop a uniform notice form for all communications to recipients about benefit changes, tax information, and other Medicaid-related matters, designed to be easy to read and written in plain language.

Mandates that mailing envelopes for notice forms include distinctive wording and colors on the front to draw recipients' attention to important Medicaid information.

Requires any third party sending information to Medicaid recipients on behalf of the division to use the same uniform notice form and mailing envelope as the division.

Authorizes managed care organizations and coordinated care programs to provide direct notice to beneficiaries about changes in benefits or services.

Effective July 1, 2016.
