Authorizes the State Department of Health to implement Rap-Back criminal history monitoring system for ongoing employee screening at licensed adult care facilities and authorizes fees as established by the State Board of Health.

Sets licensure fees for adult day care facilities at $400 plus $20 per licensed capacity (maximum $500), with annual renewal fees on the same schedule.

Establishes Medicaid reimbursement rates for adult day health services: Level I at $50/day, Level II at $75/day, Level III at $125/day, with additional transportation reimbursement of $12.50 one-way or $25 round trip for Levels II and III.
