Creates the Underground Facilities Damage Prevention Board with 16 members representing utilities, excavators, government agencies, and industry groups, with appointments due by July 31, 2016.

Establishes civil penalties for violations ranging from mandatory training for first offense to up to $5,000 for gross negligence or willful misconduct, with penalties deposited into a new Underground Damage Prevention Fund.

Exempts routine maintenance activities on publicly maintained roadways and rights-of-way from excavation notification requirements if work stays within public right-of-way and does not penetrate more than 12 inches deep.

Adds new definition for "Design Information Request" allowing professionals to request utility location information for planning purposes without intent to excavate, with operators required to respond within 7 working days.

Establishes the Underground Damage Prevention Fund to be administered by the board for enforcement, investigations, training, and damage prevention programs, with funding from civil penalties, assessments, and grants.
