Creates a conscientious beliefs exemption from vaccination requirements for school attendance by allowing parents or guardians to submit a certificate of exemption.

Requires parents or guardians claiming exemption to complete a notarized affidavit on a form provided by the State Department of Health that acknowledges understanding of vaccination benefits and risks.

Allows parents to specify which particular vaccinations the exemption applies to and requires submission of a new exemption form each school calendar year, with exemptions expiring one year from the date signed.

Exemption forms are available by written request to the State Department of Health or county health department, or by downloading from the State Department of Health website.

Takes effect July 1, 2017.
