Requires the exemption letter to include specific statements that the parent/guardian has been informed by a healthcare practitioner and school district about vaccination risks and benefits, and acknowledges understanding of vaccination consequences.

Permits state or local health officers to remove unvaccinated exempt children from school during disease outbreaks and prevents their return until outbreak resolution is approved.

Maintains existing medical exemptions from licensed physicians and allows up to 90 days for children to complete vaccinations if not exempt, with suspension required after that period for non-compliance.

Takes effect July 1, 2017.
