Prohibits health insurers from discriminating against any willing health care provider located within the plan's geographic coverage area who accepts the insurer's terms and conditions for participation.

Bans health insurers from imposing monetary advantages or penalties (higher copayments, reduced reimbursement, or provider promotion) that would influence beneficiaries' choice among participating providers.

Requires health insurers to establish relevant, objective standards for provider participation that account for case mix, severity of illness, and patient demographics, and prohibits excluding providers based on geographic location or patient risk profiles.

Authorizes the Commissioner of Insurance to enforce any willing provider laws and seek injunctions against violating insurers; allows individuals adversely affected to sue for injunctive relief.

Applies to all health insurers, multiple employer welfare arrangements, and multiple employer trusts; creates exceptions for noninsurer networks and employer-contracted provider networks; effective July 1, 2017.
