Requires the State Board of Medical Licensure to prepare and approve a standard medical referral form for physicians to use when referring patients to other physicians.

Mandates all licensed Mississippi physicians provide fully completed medical referral forms to receiving physicians at the time of referral in both written and electronic form.

Medical referral forms must include current medications, allergies, major medical history, presenting symptoms, reason for referral, and any other information required by the board.

Authorizes the State Board of Medical Licensure to monitor compliance and impose disciplinary action on physicians who consistently fail to comply with the referral requirements.

Effective date: March 26, 2017.
