Defines "post-acute residential brain injury rehabilitation facility" as a facility with no more than 12 beds providing medically directed long-term, nonacute rehabilitation for patients with acquired brain injury.

Requires post-acute residential brain injury rehabilitation facilities to be located at least 25 miles from the nearest acute care rehabilitation hospital and at least 5 miles from municipalities with populations of 10,000 or more.

Mandates that facilities obtain a license from the State Department of Health and prohibits them from participating in the Medicaid program at any time.

Makes the non-participation in Medicaid a binding condition of licensure that carries forward to any subsequent owners, with license revocation as penalty for violation.

Effective July 1, 2017.
