Directs the Division of Medicaid to select a provider-sponsored health plan to provide Medicaid services on a capitated basis under its managed care program, in addition to existing contracted entities, effective January 1, 2019.

The selected provider-sponsored health plan must be licensed as a health maintenance organization or insurance company by October 1, 2018, and cannot have ownership interest from any currently contracted managed care entities.

Establishes a five-year pilot program beginning January 1, 2019, with the provider-sponsored health plan covering at least 20% of the population enrolled in the managed care program.

Requires the Division of Medicaid to submit annual reports to legislative leadership starting December 2020 comparing the provider-sponsored health plan's performance to other plans in areas including health outcomes, administrative costs, provider satisfaction, and provider participation.

Extends the repeal date of Section 43-13-117 from June 30, 2018, to July 1, 2021.
