HB 1361 - Medicaid Health Homes High Utilizers Pilot Program

• Creates a pilot program administered by the Division of Medicaid to identify and serve high-risk, high-cost Medicaid beneficiaries at greatest risk for high hospital utilization in Hinds, Madison, and Rankin counties.

• Requires the pilot program to provide intensive targeted case management for individuals with multiple chronic conditions, mental illness, and substance use disorders, with partnerships to social service providers including homeless centers, legal services, and charity care organizations.

• Directs a work group consisting of representatives from Jackson State University School of Public Health, University of Mississippi Medical Center, Division of Medicaid, State Department of Health, managed care organizations, Jackson Hinds Comprehensive Health Center, and The Mississippi Bar to develop recommendations by December 1, 2019, with program implementation by March 1, 2020.

• Amends Section 43-13-117 of the Mississippi Code to add targeted case management and services provided under the pilot program as a covered Medicaid service.

• Takes effect July 1, 2019.
